New Patient Information
There is one step to making an appointment it will take about 10 minutes
(get your health care insurance cards out before you start )
This is a two step process
Step 1: Online registration" This will speed your appointment
Step 2: Your history ( this will allow me to classify the appointment time and urgency) If you have hives, chronic cough, sinusitis, drainage please revisit the home page for additional information. In the case of hives or swelling, there is (another) questionnaire to fill out get to it after you submit the history forms- relax this will save a ton of time for you and reduce the number of office visits.

We are acutely aware and sensitive of the time and investment you have in visiting with us. We really try to keep the initial costs of visiting a specialist down by doing the minimal number of tests possible. We will give you a copy of your records with each visits, show you how to research your own problems on the internet, and partner with you to make you independent quickly. Write your questions down I welcome hand held recording devices anything that will help you to save time and improve communications
Please plan on spending at least 1.5 hours in this visit.
The exam rooms are small, please limit the number of people you bring to the
absolute minimum - if possible, place other children with alternative care so
you may devote attention to the consultation. Feel free to bring a
recording device if there are other family members who want to hear what is
YOU MUST HAVE A PRIMARY CARE PHYSICIAN
I believe in the concept of a central managing person - a primary care doctor. I am merely a consultant. I do not have laboratory or X-ray equipment in the building which is necessary for efficient primary care. If you don't have one, we can assist you in choosing one and visiting them when you are ill. As a team, we can put together a plan for your care.
BE ON TIME FOR YOUR APPOINTMENT - WE DON'T USUALLY RUN LATE
Make plans to be here a bit early - if you are late, we may have to reschedule - no kidding. A small plastic device pictured below will be lightly pressed onto the forearm. You can expect only ten tests - one device. Positive reactions which look and feel like mosquito bites occur in 15 minutes
Its a little tricky to find us the first time especially with all the construction. We are in the Crosslands Plaza, on the corner of Crosslands and Hwy183 (which is also called Southwest Blvd). We are on the access road north of 183. The building is labeled 6310 on the top/front.
Coming from Dallas on I-20/820:Exit Bryant Irvin but with all the construction, the Bryant Irvin exit for 183 is intermittently open/closed. You may have to exit at Hulen and stay on the access road. The access road is one way. Go straight through the red light at Bryant Irvin, staying on the access road. You will pass Ajax Glass on the right then you will come to a stop sign. Turn right at the stop sign and take an immediate right into the parking lot. Go all the way to the end of the 2 buildings. Our building has 6310 on it. Park by the flags and when you enter the front door, the elevator is immediately in front of you. Go up to second floor, turn right and you will see the office, Suite 200.
Coming from Weatherford on I-20/820: Exit Bryant Irvin and turn left to go over highway. Be aware because the far left lane is turn only and will put you back on I-20 going west - the middle lane is best. Staying on Bryant Irvin, go under 183 and turn left onto access road. On the access road, you will pass Ajax Glass on the right then you will come to a stop sign. Turn right at stop sign and take an immediate right into the parking lot. Go all the way to the end of the 2 buildings. The building has 6310 on it. Park by the flags and when you enter the front door, the elevator is immediately in front of you. Go up to second floor, turn right and you will see the office, Suite 200.
