Patient Information
There is one step to making an appointment
– it will take about 10 minutes –
(get your health care insurance cards out before you
start )
This is a two step process
Step
1: Online registration" This will speed your appointment
Step
2: Your history (
this will allow me to classify the appointment time and urgency) If you have
hives, chronic cough, sinusitis, “drainage” – please revisit the home page for
additional information. In the case of hives or swelling, there is
(another) questionnaire to fill out – get
to it after you submit the history forms-
relax – this will save a ton of time for you and reduce the number of
office visits.

We are acutely aware and sensitive of
the time and investment you have in visiting with us. We really try to keep the initial costs of visiting
a specialist down by doing the minimal number of tests possible. We will give you a copy of your records with
each visits, show you how to research your own problems on the internet, and
partner with you to make you independent quickly. Write your questions down – I
welcome hand held recording devices – anything that will help you to save time
and improve communications
Please plan on spending at least 1.5 hours in this visit.
The exam rooms are small, please limit the number of people you bring to the
absolute minimum - if possible, place other children with alternative care so
you may devote attention to the consultation. Feel free to bring a
recording device if there are other family members who want to hear what is
said.
YOU MUST HAVE A PRIMARY CARE PHYSICIAN
I believe in the concept of a central managing person - a primary care doctor.
I am merely a consultant. I do not have laboratory or X-ray
equipment in the building which is necessary for efficient primary care.
If you don't have one, we can assist you in choosing one and
visiting them when you are ill. As a team, we can put together a plan for
your care.
BE ON TIME FOR YOUR APPOINTMENT - WE DON'T USUALLY RUN LATE
Make plans to be here a bit early - if you are late, we may have to
reschedule - no kidding. A small plastic
device pictured below will be lightly pressed onto the forearm. You can
expect only ten tests - one device. Positive reactions which look
and feel like mosquito bites occur in 15
minutes

It’s
a little tricky to find us the first time.
We are on the corner of Crosslands and Hwy183 which is also called
Southwest Blvd. We are on the access
road behind the large Sam’s store which is the corner of 183 and Bryant Irving
Blvd. The access road is one way. You have to go west of Sams, then come back
under the freeway. If coming from the
Dallas side, take the Bryant Irving exit and go through the light, and turn
right on Crosslands.
